How to use the Roster Template Below

The Roster Template is a Microsoft Excel (MS Excel) worksheet. Simply download the file, open it in your MS Excel, complete the information requested and save the worksheet. Once saved you will EMail it to affiliate@cdc-ca.info where it will be processed.

IF you do not have MS Excel the template will NOT work on your computer. In this event, list your members in text file with commas separating the fields, ie.:
Officer/Member, Name, Street Address, City, State, Zip, Phone, Email, Comments;
Officer/Member, Name, Street Address, City, State, Zip, Phone, Email, Comments;
NOTE the “;” at the end of each individuals record.

When this listing is complete, email it to affiliate@cdc-ca.info
